READ CAREFULLY - THIS AFFECTS YOUR LEGAL RIGHTS. In exchange for participation in the activities of EV Soccer League including but not limited to tryouts, practice, and games organized by EV Soccer League or its affiliates, I agree for myself, the members of my family, and invited guests to the following: 1. ASSUMPTION OF THE RISKS AND RELEASE. I recognize that there are certain inherent risks associated with the above described activity and I assume full responsibility for personal injury to myself and (if applicable) my family members, and further release and discharge EV Soccer League, its parent company, its owners, its staff, contractors, and affiliates for injury, physical or material loss, or damage arising out of me, my family, or invitee’s participation or attendance whether caused by the fault of myself, my family, EV Soccer League and its affiliates, or other third parties. 2. AGREEMENT TO FOLLOW DIRECTIONS. I agree for myself, my family, and other invited guests to observe and obey all posted rules and warnings posted online at https://evsoccerleague.com/league-rules/ and further agree to follow any oral instructions, written instructions, or directions given by EV Soccer League, its employees, representatives, or agents of EV Soccer League. 3. WITH UNDERSTANDING. My invitees, my family and I are agreeing to participate or be present in EV Soccer League’s events and activities with the understanding that all the coaches, team staff, and contractors that me or my family are playing for or will frequently be around are considered independent and not a part of the EV Soccer League organization. I understand that EV Soccer League is strictly considered the host of these events or activities. 4. PRESENCE AND LIKENESS. I agree to allow EV Soccer League and its affiliates to post content that may contain me or my family’s pictures or likeness that are attending or participating in EV Soccer League events or activities. I understand that some of this content can be made for profit in the benefit of EV Soccer League, its partners, and affiliates. 5. INDEMNIFICATION. I agree to indemnify, make harmless, and defend EV Soccer League, its parent company, its owners, its contractors, and its affiliates against all claims, causes of action, damages, judgments, costs or expenses, including attorney fees, legal fees and other litigation costs, which may in any way arise from me or my family's presence or participation in any EV Soccer League event. 6. FEES. I agree to pay for all damages to EV Soccer League or the owners of the facilities where EV Soccer League chooses to hold its events caused by any negligent, reckless, or willful or accidental actions by me, my family, or invited guests. By completing this application you are stating that you understand and agree to all of the information on these terms and conditions.
